If you have a general message, or if you are unsure who to send your message to, please address it to info@guidedog.org, and we will promptly route it to the appropriate person.

Guide Dog Foundation is committed to providing quality service and working in an open and accountable way that builds trust and respect. Should a constituent have a grievance or complaint, Guide Dog Foundation provides the opportunity for the individual or group to present their complaint. Guide Dog Foundation will make every effort to resolve all disputes satisfactorily within a reasonable timeframe.

We encourage constituents that have a complaint or grievance to first address the matter with the appropriate department manager or supervisor (Department contact information can be found below). In the event that an issue cannot be resolved at that level, then the process can move to the next step.

You may submit a grievance, complaint, or problem in writing by mailing a certified letter to our campus at 371 East Jericho Turnpike, Smithtown, NY 11787, Attention: Executive Office or by emailing us at executiveoffice@guidedog.org. A copy of our complaint policy is available upon request.
